Funded by the Florida Blue Foundation, the Bridges to Better Care: Training Health Leaders for All Communities Program is developed and implemented by consortium partners, Orlando Health, Moffitt Cancer Center, and United Way of Broward County. The partners represent a statewide, cross- collaboration of health touchpoints, including a system, a specialty care hospital and a community catalyst. Within each partner organization is a team of diverse, highly experienced, subject matter experts, advocates, and advisors.
To further support positive health outcome efforts in our community, the purpose of the Bridges to Better Care Champion Micro-grants is to provide funding to organizations to advance positive health practices in their own organizations. The aim is to provide up to $5,000 to organizations to take on small, health outcome-related projects in their community and/or to help support the conducting of the Bridges to Better Care: Training Health Leaders for All Communities at their home organizations. Examples of appropriate projects include (but are not limited to) requesting funding to translate organization documents into another language, room reservations/meeting expenses to hold the program training at their facility, requesting outside hours/FTE support for staff to take training, and funding to support a health outcome-focused community events.
